Children's Case Management Program
ÂCase Managers are Licensed Social Workers who are experienced in family centered services. We will work with families to identify and address any needs connected to a child’s Mental Health, (including a substance related disorder), Mental Retardation, or an Autism diagnosis.
Case managers will assist families in developing and maintaining family teams, creating crisis plans, creating individualized service plans, advocating, and accessing community services and other resources.
We welcome referrals from family members, the medical community, individuals, case managers, DHHS, local hospitals, and all other service providers.
ÂFamilies have a choice of case management agencies.
Community Concepts’ Children’s Case Management Program provides several services:
- Locating and assessing services.
- Identifying goals and developing a service plan to achieve them.
- Attending meetings at school to help advocate.
- Coordination with other service providers
- Building and fostering natural and community based supports.
Children and their families will receive services at a level appropriate to their Mental Retardation, Mental Health, (including a substance related disorder), or Autism needs in a coordinated fashion with their other providers.
This service is covered by Maine Care, through the: Maine Department of Health and Human Services.
